Activities of daily living (ADL’S)

 Support coordination is about supporting you to understand your NDIS plan and the funding components and how you want support to achieve your goals. We work with individuals and their formal and informal supports to work towards the goals in a way you want and need.

 

  • Understanding and connecting – planning and scheduling – monitoring and advocacy and reporting

 We are here to support you to be encouraged in all areas of your life no matter your level of support required.  We will do this with do this with a person-centred practices approach in the following areas.


 

  • Personal Care – light jobs around the home – support with menu planning, cooking, and shopping – support with medical appointments, medication administration and collection – budgeting.

Social and community access

Short term accommodation (STA )

 Having a disability can be very isolating and we are here to support and encourage everyone to engage with their community in ways that they find meaningful.  With our person-centred approach, we work with you on what you are wanting to achieve with your goals and level of engagement.  

  • Social groups participation – local activities and events – special interest groups (hobbies) – support to visit family and friends – recreational activities – transport.

Short term accommodation (STA )

 Short term accommodation or STA will be listed in your NDIS plan this is to allow for you to have a break from your usual careers or living situation.  We will work with you to plan your ideal break, and this includes where and what you will do.  Please contact us to discuss further.

 We will work with you on the skills you are wanting to learn, like budgeting, cooking, life skills and transport training.
